The undersigned individual (parent or guardian), represents that the registrant is in good health and can participate in volleyball and associated activities and with prior knowledge of the physical nature of these activities as well as the inherent risk of exposure to contagious diseases, releases Premier Volleyball Club and its coaches/staff, from any and all responsibility for injury to the registrant through negligence or otherwise while she is participating in these activities. The Parent or Guardian assumes all risks inherent to these activities and will hold Premier Volleyball Club and its coaches/staff harmless from any and all claims and/or causes of action that may arise from these activities.
